Deferred Members Withdrawal Form
This form is for deferred members only who want to take a full or partial withdrawal
Cease or Vary Contributions Form
This is the form that allows a member to either cease their contributions to the Scheme or alter the percentage of their salary that they currently contribute
Investment Switch Form
This is the form that allows a member to transfer their current account balances and invest all their future member and employer contributions into a different investment fund (either the Balanced Fund or the Cash Fund) than the one they are currently in.
Nominated Beneficiary Form
This is the form that allows a member to elect or to change those who are nominated as beneficiaries in the event of the death of a member. The information on the form will be used by the Trustee to inform them as to the payment of a death benefit from the Scheme.
Benefit Request Form
This is the form that allows a member to obtain a benefit from the Scheme in the event of a member leaving service, becoming incapacitated, being made redundant or retiring. The information on the form will be completed by the member and the employer and be used by the Administration Manager to make the benefit payment.
Property (Relationships) Act 1976 Hardship Guidelines
The Trustee may permit a Property (Relationships) Act 1976 Hardship benefit to be paid in terms of Clause 10 of the Trust Deed

This form is to apply for a Property (Relationships) Act 1976 hardship benefit.
Standard Financial Hardship Guidelines
The Trustee may permit a financial hardship benefit to be paid under Clause 8A of the Trust Deed.
Standard Financial Hardship Application Form
This form is to apply for a standard financial hardship benefit.
